We are thrilled you have chosen to establish your care with us! Below you will find information about establishing as a new patient with our medical department, our dental department, or both. To get started, please download the 2020 New Patient Registration Packet below. Each form of the packet must be completed in black ink and delivered either by mail or in person to any of our Health Center Locations. After we process your paperwork we will contact you within 5 business days to set up your first appointment.
New Patient Packet
Should you need immediate care before your first visit with us, please contact your former provider or the nearest emergency room. If you will need medication refills during the period of transferring to us please contact your former provider to let them know. After you have been seen by one of our providers we will be happy to support your care in all the ways we can.
Before your first visit with us, if you have insurance please send a copy of your card and take any steps necessary to ensure your coverage is valid. If you currently do not have insurance, please let us know and we will schedule a free appointment with one of our Certified Application Counselors to help you apply for affordable insurance or pay for services on a sliding fee scale that is based on your income.
