Community Health Center of Franklin County Services:
Medical Services:
The Health Center offers high quality primary care, with an emphasis on preventive care and education. Our patients are offered a full range of primary care services, including physical exams, gynecological exams, urgent care, disease management, diagnostic screenings, and laboratory services.
Dental Services:
The Health Center provides preventive, restorative, and emergency dental care for children and adults. Services include exams, cleanings, fillings, extractions, sealants, and other general dental procedures. Particular emphasis is placed on preventive services for children.
Mental Health Services:
The Health Center has mental health personnel who provide individual and group counseling and case management services for our primary care patients of all ages.
Outreach:
The Health Center provides Outreach to Franklin County communities to assist people in accessing health care.
Tobacco Cessation Group:
We offer a weekly support group facilitated by a trained, bilingual Tobacco Cessation Counselor.
Town Nurse Program:
Health center nurses provide services to Franklin County and North Quabbin communities by conducting home visits, nursing visits, health screenings, and educational activities in the towns we serve.
Farmworkers:
As a partner in the Connecticut River Valley Farmworker Program, we provide transportation, translation, and evening hours so that all of our services are accessible to the area’s migrant and seasonal farmworkers.
Children and Families:
We offer a range of special services to make it easier for all children to have the healthiest start possible. These services include childhood immunizations, the Reach Out and Read program to promote early literacy, and on site WIC services.
24 Hour Emergency Coverage:
Our providers offer telephone and hospital coverage for our patients 24 hours a day, 365 days a year.
Enrollment Assistance:
We are happy to assist our patients with applying for public health programs such as MassHealth, Commonwealth Care, Children’s Medical Security Plan, and Free Care.
Interpreting:
We offer Spanish interpreting throughout the practice.
Transportation:
We work to help patients lacking private transportation so that all of our services accessible.
