Integrated Behavioral Health Services

By combining primary care and mental health care in one setting, we created a team of healthcare professionals working seamlessly and collaboratively for you. Our behavioral health clinicians work alongside primary care providers to offer on-demand consultation, brief therapy, and group therapy. Our goal is to help you achieve your goals for health and well-being, whether it is to feel better, manage life stressors and crises, or have better relationships.

We work with adults, adolescents/children, couples, and families. We treat most diagnoses, including but not limited to depression, anxiety, bipolar disorders, traumas, and substance abuse. We also work closely with our Community Health Workers (CHWs) to ensure that your social and economic needs are met.

If you have any questions, concerns, or suggestions at any time, please do not hesitate to reach out to us! Thank you for choosing the Integrated Behavioral Health Services at CHCFC!

What is brief therapy and how could it help me?

Most patients, including those with history of traumas and/or chronic mental health diagnoses, could benefit from brief, solution-focused therapy. For example, brief Cognitive Behavioral Therapy and brief Dialectical Behavior Therapy have been proven effective in helping patients recognize problematic thinking and behavioral patterns, enhance coping skills, and improve interpersonal relationships. Your clinician will help you decide whether brief therapy is right for you during the free consultation. If your concerns are more suited for long-term therapy, we will help you connect with outside mental health providers.

What is the duration and frequency of brief therapy?

After the initial consultation, eligible patients may receive up to twelve intake/individual/couples/family therapy sessions (30-45 minutes each), occurring either weekly or bi-weekly. Occasionally, your primary therapist may recommend an additional around of twelve sessions based on their clinical judgment and in consultation with the treatment team. Our program does not provide open-ended therapy. If you would like to continue therapy beyond the recommended session limit, we will provide outside referrals for you. There are no session limits for groups. All behavioral health services are provided either in-person or via telehealth.
