    Looking for help accessing food? CHW’s can help with SNAP applications, food assistance referrals, Meals on Wheels, local food pantries, mobile food banks, and other food resources.

  • Housing Support

    CHW’s can help you explore housing resources, including RAFT, CHAMP, Section 8, Coordinated Entry, shelter resources, local housing options, and housing searches.

  • Transportation


    Need help getting where you need to go? CHW’s can assist with rides, community transportation, public transportation applications, and navigating local transit options, including our own transportation for appointments.

  • Utility Assistance

    If you’re having trouble with utility costs, CHW’s can help with medical protection forms, financial hardship forms, discount rate applications, and other utility assistance programs.
